SILK CANADA RECALL PRODUCT REFUND REQUEST FORM

To request a refund or replacement coupons in relation with the Silk Canada product recall, please fill in the required fields on the following form. You will be required to provide some personal information, such as your mailing address. Your personal information will be used to process your recall request. For more information, review our Privacy Policy here: Privacy Policy (danone.ca).

    * Coupon Terms and Conditions: Each coupon is valid on the purchase of one (1) Silk beverage (6 x 236 mL, 890 mL, 940 mL, 946 mL, 1.74L, 1.75L and 1.89L formats) in Canada. Any other use constitutes fraud. Coupon is non-transferable and has no cash value. GST/PST/HST are included in the face value of this coupon, where applicable. Void if sold, transferred, reproduced or where prohibited by law. Cannot be combined with any other offer.
